Sickle Cell Awareness Group of Ontario is partnering with Ipsos Healthcare to
conduct a survey among people living with sickle cell disease.

This survey will take approximately 25-minutes to complete and will provide
valuable information towards optimizing doctor-patient interactions.

As a thank you for your participation, qualified respondents who complete the
